Jon Milenthal, Milenthal Group for Health Care Home, presentation, "Get Your Well On" Campaign.
- “Get Your Well On” is a Health Care Home campaign. The Health Care Home is a collaborative and unique approach to delivering health care to Ohio’s Medicaid recipients. The seven Ohio Medicaid Care Coordination Plans make up the Health Care Home. You can order the “Get Your Well On” materials for distribution at your facilities, and the ordering and shipping is completely free.
Jan Ritter, State Coordinator, Ohio Action for Healthy Kids provided an introduction and overview of Ohio Action for Healthy Kids.
Craig Wethington, MPH, Assistant Director, Wellness Programs, Office for Safety, Health & Nutrition, Ohio Department of Education presented Linking Physical Activity in Afterschool to the School Improvement Process.
